Pursuant to your request, please find enclosed two forms:

Fill all spaces down to and including your signature. Leave blank the spaces below the box where your signature is requested.

Petition for Affiliation by Transfer of Membership

Fill all spaces down to and including your signature, which is below the three references. The references do not need to be Masons.

Next, sign your name in the perforated section titled ‘Request for my Dimit’. I will send that to your Grand Lodge via the Grand Lodge of Florida.

On the back side of the petition there are two lines for signatures of members of Melbourne Lodge No. 143, under the word RECOMMENDATIONS. The Signatures of TWO members of Melbourne Lodge are required.

Return these two completed forms with a check for $65 made out to Melbourne Lodge No. 143 in the enclosed envelope. This check will include your dues for the current year, through December 27th.
