Pursuant to your request, please find enclosed two forms:

Applicant Background Waiver

Fill all spaces down to and including your signature. Leave blank the spaces below the box where your signature is requested.

Petition for Dual Membership

Fill all spaces down to and including your signature. On the back page of the Petition, include three references. The references do not need to be Masons. Also, have two members of Melbourne Lodge No. 143 sign as your recommenders.

Complete the TOP SECTION of the form titled Petition for Certificate of Good Standing for Dual Membership (GL604c). I will send that to your Lodge via the Grand Lodge of Florida.

Return these three completed forms with a check for $65 made out to Melbourne Lodge No. 143 in the enclosed envelope. This check will include your dues for the Masonic Year ending December 27, ____.
