Dear Brother «LAST_NAME»,

Pursuant to our phone conversation yesterday, enclosed is a Petition for Reinstatement and an Applicant Background Waiver.

Applicant Background Waiver: Please include all information down to the line for your signature and date in the text box. Do not sign the section below the box.

Petition for Reinstatement: Include all information down to the bottom with your full name signed. The signatures from two members of the Lodge must be Master Masons. Your references can be anyone who has known you for five years. Please include your check for $190 with your Petition.

Please print legibly so that I can effectively process your Petition.

When you have completed the above, please have one of your Lodge recommenders deliver the above with your check to the undersigned.

Once I receive your Petition, and after I complete the Background Check, your Petition will be officially RECEIVED by the Lodge. At that time the Worshipful Master will assign an Investigation Committee. This committee will contact you to arrange a date and time to meet with you and your wife at your home.

The earliest your Petition can be balloted upon is one lunar month after it is RECEIVED by the Lodge.
