Fill all spaces down to and including your signature, which is below the three references. The references do not need to be Masons.

Next, sign your name at the bottom of the form

Enclose your Dimit with the Petition.

Return the completed form and your Dimit with a check for $65 made out to Melbourne Lodge No. 143 in the enclosed envelope. This check will include your dues through December 27, 20__.
