Fill all spaces down to and including your signature, which is below the three references. The references do not need to be Masons.

Next, sign your name in the perforated section titled ‘Request for my Dimit’. I will send that to your Florida Lodge after your election into Melbourne Lodge No. 143, F&AM

On the back side of the petition there are two lines for signatures of members of Melbourne Lodge No. 143, under the word RECOMMENDATIONS. Please complete with these signatures.

Return this form with a check for $50 made out to Melbourne Lodge No. 143 in the enclosed envelope. This check will include your dues for the current year.
